- Provide your full legal name and your electronic or physical signature.
2. Where to Send Your Notice

3. Counter-Notification Procedure
If you receive a notification that material you posted on our site has been removed or disabled due to an infringement claim, and you believe that this material is not infringing or that you have the authorization from the copyright owner, the copyright owner's agent, or pursuant to the law, you may send a counter-notification to our Copyright Agent.
Your counter-notice must contain:
- Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material.
- Your name, address, telephone number, and email address.
- A statement that you consent to the jurisdiction of the federal court in which your address is located, or if your address is outside the United States, for any judicial district in which the service provider may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
- Your physical or electronic signature.
4. Repeat Infringers Policy
In accordance with the DMCA and other applicable laws, MVA Quote maintains a policy of terminating, in appropriate circumstances and at our sole discretion, users or accounts that are deemed to be repeat infringers.
